Current Situation
Zahra explains that she reached out to David this past Monday morning, and that he and Linda had just returned from Florida. He apologized for the misunderstanding and said that he had appointments to tour homes this week. 

Zahra also tells you that on Tuesday the Case Worker from the Local Health Integration Network, Sandra, had met with Mr. Roberts to discuss the application to Long-Term Care. Sandra was very surprised to find out that Mr. Roberts was unaware of the plan. Sandra has told the hospital team that she is very disappointed that Mr. Roberts has not been included in the discussion, and that she would be reporting this to her manager. Sandra said that she found no evidence that Mr. Roberts was incapable of making this decision for long-term care, and had accepted his refusal of long-term care. 

Mr. Roberts’ physician this week, Dr. Sawatzky, refuses to write the discharge order because she thinks that home is not a safe option for Mr. Roberts. She states that she will not take on that legal risk.

Zahra has just gotten off the phone with David and Linda. They are very angry that the LHIN approached Mr. Roberts directly. David stated that his father cannot return home, and told Zahra that he has arranged for Mr. Roberts’s home to be listed for sale, and that the water and electricity are scheduled to be shut off in the coming days.   

Zahra says there is a meeting planned for tomorrow morning. She tells you that the meeting will involve Zahra, Dr. Sawatzky, Sandra from the LHIN, David, and Linda. Zahra has asked Mr. Roberts to attend, but he refuses.
